Passport Regulations Have Changed

What is needed to obtain a passport?
 

Passport Application Form - Available online U.S. Department of State, at Bothell City Hall, or from the Bothell Municipal Court's Passport Office.

 

Proof of Citizenship - A Birth Certificate, expired Passport, or Naturalization paperwork.
 
Photo Identification - A Driver's License, Military ID, or Government-issued ID.

One Passport Photograph - These are obtainable through most locations that process photographs.

Children under the age of 16 need both parents present as well as the child applicant.

How much does a Passport cost?

Passport fees vary depending on whether citizens apply by mail or in person, and if they request the Expedite Service to receive a passport faster than normal processing time.

Two separate fees are required:

1) One payment (check or money order only) made out to 'U.S. Dept. of State' for application and expedite fee:
$110   Persons age 16 or older
$80   Persons under age 16
$60   Optional Expedite Service Fee

2) One payment (cash, check, money order,or Visa or MasterCard) made out to 'City of Bothell for execution fee:
$25  Execution Fee

What is a "U.S. Passport Card" and its cost?

The wallet-size U.S. Passport Card is convenient and less expensive than the passport book. However, the U.S. Passport Card cannot be used for international air travel. This new travel document can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry.

$15   Persons under age 16
